A 78-year-old man from Malibu, California, has died from injuries he sustained after his vehicle crossed the centerline and collided with a semi-truck on US-95 in Fallon, Nevada. The Nevada Highway Patrol confirmed that Thomas Waljeski succumbed to his injuries on November 15, four days after the crash.

The incident occurred at approximately 9:12 a.m. on November 11, 2025. According to the preliminary investigation, Waljeski was driving a 2024 red Dodge Hornet southbound on US-95, just south of Paiute Drive. For reasons under investigation, his vehicle entered the northbound lane and crashed into a 1999 Kenworth semi-truck that was towing an enclosed trailer.

First responders transported Waljeski to a local hospital with life-threatening injuries following the collision. The driver of the Kenworth was not reported to have sustained serious injuries.

The Nevada Highway Patrol’s Major Incident Reconstruction Team is leading the investigation into the crash. According to NHP data, this incident is part of 36 fatal crashes investigated by the region year-to-date, resulting in 38 fatalities. Officials note these figures are preliminary and subject to change pending final reports.
